Vaccination Requirement

HLAA has made the decision that all participants at the in-person HLAA 2022 Convention at the JW Marriott Tampa Water Street, whether a presenter, attendee, exhibitor, staff, guest, or vendor, will be required to be fully vaccinated. Participants must provide proof of vaccination to register for the in-person event.

Provide Proof of COVID-19 Vaccination

HLAA requires that all registrants send a picture or a copy of their vaccine card with both online and in-person registration forms. All information gathered will remain confidential. We have three easy options to provide your vaccination card.

  1. Email a copy or picture of your vaccine card to verify@hearingloss.org.
  2. Upload a copy or picture of your card to a secure Dropbox account here.
  3. Mail a copy or picture of your vaccine card with your paper registration form to the HLAA office.

You have 7 days from our receipt of your registration form to provide a copy of your vaccination card. If we don’t receive it within 7 days, then your registration will be rejected and refunded.

COVID-19 Personal Responsibility Statement

Anyone who registers for the in-person event will need to agree to the COVID-19 Personal Responsibility Statement as follows:

I acknowledge that HLAA cannot guarantee that I will not be exposed to or contract a Communicable Disease, such as COVID-19, at the HLAA 2022 Convention. As a participant in the upcoming HLAA Convention, I agree to abide by all established and applicable COVID-19-related requirements, advisories, policies, procedures, and protocols of the Convention, Hotels and HLAA, as well as the CDC, state of Florida and any other governmental authority for the duration of the event.​ I understand the risk that I may be exposed to or infected by a Communicable Disease by attending the HLAA Convention. I knowingly and freely assume all such risks, both known and unknown, and assume full personal responsibility for my participation. I release HLAA and their staff and contractors from liability should I become ill, directly or indirectly, as a result of my participation in the event.
